Corrective Jaw Surgery Planned by an MDS Orthodontist

Some bite and facial problems come from the jaw bones themselves, so braces alone cannot fix them. Surgical orthodontics combines orthodontic treatment with corrective jaw surgery to realign the jaws, improve your bite and bring balance to your face.

The basics

What Is Surgical Orthodontics?

Surgical orthodontics, also called orthognathic surgery, corrects problems with the structure, growth and position of the jaws. It treats bite problems caused by skeletal imbalance, and can also help with sleep apnoea, airway problems and jaw joint (TMJ) disorders linked to jaw position.

Crooked teeth and bad bites are normally corrected by an orthodontist, while jaw abnormalities are corrected by an oral and maxillofacial surgeon. When both are present, the two specialists work together. At FDOC, this team approach gives adults and children with complex dental and facial problems a complete, coordinated treatment plan.

Who needs it

Conditions Treated with Surgical Orthodontics

Surgery is recommended when the jaw discrepancy is too large to correct with braces or aligners alone.

Severe underbite

Lower jaw sits too far forward of the upper jaw.

Severe overbite

Upper jaw projects well beyond the lower jaw.

Open bite

Front teeth do not meet when the back teeth bite together.

Facial asymmetry

Jaws or chin noticeably uneven from one side to the other.

Receding chin

A small or set-back lower jaw affecting your profile.

Sleep apnoea

Airway problems linked to jaw position and size.

Jaw joint (TMJ) problems

Pain or clicking linked to how the jaws fit together.

Difficulty chewing or speaking

Everyday function affected by the jaw relationship.

Team approach

Two Specialists, One Treatment Plan

Surgical orthodontics works best when the orthodontist and surgeon plan every step together.

Orthodontist

Diagnoses the bite, plans tooth movement and fits braces before and after surgery.

Oral & maxillofacial surgeon

Repositions the upper jaw, lower jaw or chin during the operation.

Joint planning

Records, X-rays and 3D scans are used to plan the final bite and facial profile.

Follow-up together

Both specialists track healing and fine-tune the bite until treatment is complete.

Step by step

How Surgical Orthodontic Treatment Works

Treatment runs in stages, with braces before and after the operation and retainers at the end.

1

Consultation & records

Examination, X-rays, photos and 3D scans to assess the teeth, jaws and face.

2

Pre-surgical orthodontics

Braces align the teeth so they fit together once the jaws are repositioned.

3

Surgical planning

The orthodontist and surgeon plan the jaw movements and final bite together.

4

Jaw surgery

The maxillofacial surgeon repositions the jaw in hospital under general anaesthesia.

5

Recovery & post-surgical orthodontics

After healing, braces fine-tune the bite into its final position.

6

Retainers

Braces come off and retainers keep the new bite stable long term.

Surgical orthodontics, or orthognathic surgery, combines braces with corrective jaw surgery to fix bite and facial problems caused by the position or size of the jaws.

An orthodontic assessment with X-rays and 3D records shows whether the problem is in the teeth or the jaws. Braces or aligners can correct mild to moderate bite problems; large skeletal discrepancies usually need surgery.

The operation is performed by an oral and maxillofacial surgeon. The orthodontist plans the treatment with the surgeon and handles the braces before and after surgery.

Yes. Braces are usually worn before surgery to align the teeth and after surgery to fine-tune the bite, followed by retainers once the braces come off.

Jaw surgery is generally done once facial growth is complete, which is usually in the late teens or later. Your orthodontist will confirm the right timing for you.

The operation is done under general anaesthesia, so you feel nothing during it. Swelling and discomfort afterwards are managed with medication and usually ease over the following weeks.

Moving the jaws often improves facial balance, chin position and side profile. Your specialists will discuss the expected changes with you during planning.

In some cases, yes. When sleep apnoea, airway problems or jaw joint (TMJ) pain are linked to jaw position, correcting the jaws can help.
